The Gravity Forms extension for Download Monitor lets you require users to complete a Gravity Forms form before accessing any download. Easily attach forms to specific downloads, ensuring that content is only available once the form is successfully submitted.

With a simple shortcode, you can display forms anywhere on your site, giving you full flexibility to design custom download pages. All submitted data is stored within Gravity Forms, and entries can be easily exported to CSV files. Note: this extension requires the Gravity Forms plugin to be installed and activated.
